Survey Approval Request Form
Thank you for taking the time to complete the Survey Approval Request Form. The MSU Survey Committee was created to manage the flow of surveys to all members of the MSU community. The committee is responsible for reviewing all surveys within scope and purview of the committee. It also manages and maintains an institutional survey calendar in order to:
- Reduce campus survey fatigue.
- Avoid duplication and seek to improve collaboration.
- Prevent over sampling.
- Coordinate release dates prioritizing institution wide surveys.
- Provide calendar with list of upcoming and completed surveys.
- Improve survey quality and collection of information.
- Review surveys for sensitive information.
- Check for the level of detail requested for demographic and personal information.
- Review process for administering survey and storing data.
It is important to note that certain dates are reserved for university-sponsored surveys. No additional surveys can be sent out during these time periods:
- August 15th - August 31st
- October 15th - November 15th
- Last week of each academic semester
